Primary Duties
- Review and analyze assessment data to develop comprehensive Individual Service Plans (ISPs).
- Coordinate and attend interdisciplinary team meetings.
- Prepare Semi-Annual Progress Reports and maintain accurate case records.
- Train and support direct care staff in goal implementation and documentation.
- Provide job coaching support as needed.
- Ensure all documentation complies with confidentiality policies and regulatory standards.
- Monitor client progress and adjust services to meet evolving needs.
- Serve as a liaison between clients, their families, and service providers.
- Conduct required field visits with clients.
- Undertake additional responsibilities as assigned.

Requirements
- A Bachelor's degree is mandatory; a field in Human Services is preferred.
- Two years of experience drafting and implementing vocational plans in a program setting is preferred.
- Demonstrated experience supporting individuals with developmental disabilities.
- Experience designing behavior modification intervention services is advantageous.
- CPR and First Aid certification (must be obtained within the first 30 days of employment).
- A clear criminal history record as specified by DDS licensing standards (LIC 508).
- Access to dependable transportation with a valid California driver's license.
- Current automobile insurance meeting state minimum requirements.
- Firm understanding of behavior modification principles and human development.
- Superior organizational skills and the ability to work autonomously.
- Exceptional written and verbal communication skills in English (proficiency in Spanish is a beneficial addition).
- Proven ability to collaborate effectively as part of an interdisciplinary team.
