Our social services agency is seeking a dedicated and empathetic Case Manager to support adults with developmental disabilities. This is a full-time professional role where you will coordinate services, develop individualized plans, and serve as a key advocate for clients working towards greater independence. Primary Duties

  • Review and analyze assessment data to develop comprehensive Individual Service Plans (ISPs).
  • Coordinate and attend interdisciplinary team meetings.
  • Prepare Semi-Annual Progress Reports and maintain accurate case records.
  • Train and support direct care staff in goal implementation and documentation.
  • Provide job coaching support as needed.
  • Ensure all documentation complies with confidentiality policies and regulatory standards.
  • Monitor client progress and adjust services to meet evolving needs.
  • Serve as a liaison between clients, their families, and service providers.
  • Conduct required field visits with clients.
  • Undertake additional responsibilities as assigned.

Requirements

  • A Bachelor's degree is mandatory; a field in Human Services is preferred.
  • Two years of experience drafting and implementing vocational plans in a program setting is preferred.
  • Demonstrated experience supporting individuals with developmental disabilities.
  • Experience designing behavior modification intervention services is advantageous.
  • CPR and First Aid certification (must be obtained within the first 30 days of employment).
  • A clear criminal history record as specified by DDS licensing standards (LIC 508).
  • Access to dependable transportation with a valid California driver's license.
  • Current automobile insurance meeting state minimum requirements.
  • Firm understanding of behavior modification principles and human development.
  • Superior organizational skills and the ability to work autonomously.
  • Exceptional written and verbal communication skills in English (proficiency in Spanish is a beneficial addition).
  • Proven ability to collaborate effectively as part of an interdisciplinary team.
