Primary Duties

  • Examine and interpret assessment information to formulate thorough Individual Service Plans (ISPs).
  • Organize and participate in meetings with interdisciplinary teams.
  • Compile Semi-Annual Progress Reports and ensure case records are kept accurate and up-to-date.
  • Instruct and assist direct care staff with implementing objectives and maintaining proper documentation.
  • Offer job coaching assistance as required.
  • Guarantee all records adhere to confidentiality policies and regulatory requirements.
  • Track client advancement and modify services to address changing needs.
  • Act as a point of contact connecting clients, their families, and service providers.
  • Conduct necessary field visits with clients.
  • Fulfill additional duties as assigned.

Required Qualifications

  • A Bachelor's degree is necessary, with a preference for studies in Human Services or a related field.
  • Two years of prior experience drafting and executing vocational plans within a program environment is preferred.
  • Background in creating and delivering behavior modification intervention services is advantageous.
  • Direct experience supporting individuals with developmental disabilities.
  • CPR and First Aid certification (must be obtained within the first 30 days of employment).
  • A clear criminal history record as specified by DDS licensing standards (LIC 508).
  • Access to a dependable vehicle and a valid California driver's license.
  • Current automobile insurance that satisfies state minimum requirements.
  • Firm understanding of behavior modification principles and human development.
  • Superior organizational skills and the capacity to work autonomously.
  • Exceptional written and spoken English communication abilities (Spanish language skills are a bonus).
  • Proven ability to collaborate productively as a member of an interdisciplinary team.
