Health Navigator Program

Receive genuine support for your health and daily life. If you are a Medi-Cal member and find it challenging to manage medical appointments or everyday necessities, you might be eligible for a complimentary health navigation program. This program provides you with a dedicated Health Navigator—a personal advocate who listens, offers support, and assists in simplifying your life.

Overview

This is authentic, one-on-one assistance—not a call center. You will have a consistent contact who understands your situation, aids in organization, and supports you throughout your journey. The program includes monthly gift card assistance for active, engaged participants.

Your Health Navigator can assist you by:

• Scheduling appointments with doctors and specialists.
• Providing follow-up support after hospital discharges.
• Connecting you to resources for housing, food, and transportation.
• Linking you to mental health or substance use support services.
• Helping you maintain focus on your health and personal objectives.

Eligibility Criteria

If you have Medi-Cal, you likely qualify. You may be eligible if any of the following apply to you:

• Frequent emergency room or hospital visits.
• Unstable housing, currently unhoused, or concerned about housing.
• Experiencing stress, anxiety, or feeling overwhelmed.
• Facing challenges with housing, food, or transportation.
• Living with chronic or serious health conditions.
• Recently released from jail, foster care, or a facility.
• Managing mental health or substance use challenges.
• Pregnant or having recently had a baby and needing additional support.
